    The Effect of Educating Health Promotion Strategies on Self-Care Self-Efficacy in Patients Undergoing Kidney Transplantation: A Double Blind Randomized Trial

    Background: Self-care self-efficacy in patients with end stage renal disease, waiting for kidney transplantation, probably decreases due to facing new conditions and side effects of treatment. Objectives: The current study was conducted to investigate the effect of educating health promotion strategies on self-care self-efficacy in patients undergoing kidney transplantation. Patients and Methods: A double blind randomized clinical trial was conducted on 52 patients undergoing kidney transplantation in Baqiyatallah Hospital in 2012. Patients were randomly assigned into intervention and control groups. The questionnaire of Strategies Used by People to Promote Health (SUPPH) was employed to measure self-care self-efficacy. At first, the two groups filled the questionnaire. Then, the intervention group was trained regarding health promotion strategies within 4 sessions before the transplantation. The control group was trained according to routine protocol of the transplantation unit. Then, the two groups were followed up for two months, and reassessed at the end of the first and second months after the transplantation. The data were analyzed by descriptive and analytic statistics including independent samples T test, Chi square and repeated measures ANOVA. Results: In the intervention group, the mean of total self-care self-efficacy was 106.96 ± 25.1 at first, and changed to 135.81 ± 9.65 and 111.19 ± 12.45 after the first and second post-test respectively (P = 0.001). In the control group, the mean of total self-care self-efficacy was 112.73 ± 14.33 at first, and changed to 118.58 ± 17.59 and 108.73 ± 15.93 after the first and second post-test respectively (P = 0.001). Significant differences were observed between the two groups in the first post-test regarding total score of self-care self-efficacy (P = 0.001) and dimensions of reduction of stress (P = 0.001), enjoying life (P = 0.01), and coping (P = 0.001). The mean scores of the intervention group were higher than those of the controls in all domains of self-care self-efficacy in the second post-test. However, the difference was only significant in decision-making dimension (P = 0.04). Conclusions: Educating health promotion strategies was effective in improving self-care self-efficacy in patients undergoing kidney transplantation. Establishment of a holistic caring program is suggested to integrate the pre-transplantation educations with a continual post discharge follow-up

    A Closer Look at How Self-Talk Influences Skilled Basketball Performance

    Empirical literature addressing the effectiveness of self-talk for expert performers is lacking. We addressed this shortcoming within the existent literature and examined the comparative effects of instructional and motivational self-talk on basketball free throw shooting accuracy and salient movement kinematics. We recruited 20 professional basketball players to participate in a 2 × 2 pre/post-test experiment. Free throw accuracy and movement patterns were recorded, with the latter subsequently used to calculate elbow–wrist coordination variability. Results indicated superior shooting accuracy and reduced movement coordination variability for instructional self-talk compared to baseline conditions, whereas no differences emerged for motivational self-talk. Findings from the study help practitioners to better guide skilled performers how best to use self-talk; an area in urgent need of further research.</jats:p

(The latter is the order four automorphism U mapped to V, V mapped to U^{-1}, where U, V are the canonical unitaries generating A_theta.) Further, the projection e is approximately central, the cut down algebra eA_theta e contains a Fourier invariant q x q matrix algebra whose unit is e, and the cut downs eUe, eVe are approximately inside the matrix algebra. (In particular, there are Fourier invariant projections of trace k|qtheta-p| for k=1,...,q.) It is also shown that for all theta the crossed product A_theta rtimes Z_4 satisfies the Universal Coefficient Theorem. (Z_4 := Z/4Z.) As a consequence, using the Classification Theorem of G. Elliott and G. Gong for AH-algebras, a theorem of M. Rieffel, and by recent results of H.     General Relativistic Contributions in Transformation Optics

    One potentially realistic specification for devices designed with transformation optics is that they operate with high precision in curved space-time, such as Earth orbit. This raises the question of what, if any, role does space-time curvature play in determining transformation media? Transformation optics has been based on a three-vector representation of Maxwell's equations in flat Minkowski space-time. I discuss a completely covariant, manifestly four-dimensional approach that enables transformations in arbitrary space-times, and demonstrate this approach for stable circular orbits in the spherically symmetric Schwarzschild geometry. Finally, I estimate the magnitude of curvature induced contributions to satellite-borne transformation media in Earth orbit and comment on the level of precision required for metamaterial fabrication before such contributions become important.Comment: 14 pages, 3 figures.     Single synapse glutamate imaging reveals multiple levels of release mode regulation in mammalian synapses

    Mammalian central synapses exhibit vast heterogeneity in signaling strength. In order to understand the extent of this diversity, how it is achieved, and its functional implications, characterization of a large number of individual synapses is required. Using glutamate imaging, we characterized the evoked release probability and spontaneous release frequency of over 24,000 individual synapses. We found striking variability and no correlation between action potential-evoked and spontaneous synaptic release strength, suggesting distinct regulatory mechanisms. Subpixel localization of individual evoked and spontaneous release events reveals tight spatial regulation of evoked release and enhanced spontaneous release outside of evoked release region. Using on-stage post hoc immune-labeling of vesicle-associated proteins, Ca(2+)-sensing proteins, and soluble presynaptic proteins we were able to show that distinct molecular ensembles are associated with evoked and spontaneous modes of synaptic release

    Uncoupling endosomal CLC chloride/proton exchange causes severe neurodegeneration

    CLC chloride/proton exchangers may support acidification of endolysosomes and raise their luminal Cl− concentration. Disruption of endosomal ClC‐3 causes severe neurodegeneration. To assess the importance of ClC‐3 Cl−/H+ exchange, we now generate Clcn3unc/unc mice in which ClC‐3 is converted into a Cl− channel. Unlike Clcn3−/− mice, Clcn3unc/unc mice appear normal owing to compensation by ClC‐4 with which ClC‐3 forms heteromers. ClC‐4 protein levels are strongly reduced in Clcn3−/−, but not in Clcn3unc/unc mice because ClC‐3unc binds and stabilizes ClC‐4 like wild‐type ClC‐3. Although mice lacking ClC‐4 appear healthy, its absence in Clcn3unc/unc/Clcn4−/− mice entails even stronger neurodegeneration than observed in Clcn3−/− mice. A fraction of ClC‐3 is found on synaptic vesicles, but miniature postsynaptic currents and synaptic vesicle acidification are not affected in Clcn3unc/unc or Clcn3−/− mice before neurodegeneration sets in. Both, Cl−/H+‐exchange activity and the stabilizing effect on ClC‐4, are central to the biological function of ClC‐3

    Oncoviruses are implicated in around 20% of all human cancers including both solid and non-solid malignancies. Epstein-Barr virus (EBV) and human papillomaviruses (HPVs) are the most common oncoviruses worldwide. Currently, it is well established that onco-proteins of EBV (LMP1, LMP2A, and EBNA1) and high-risk HPVs (E5 and E6/E7) play an important role in the initiation and/or progression of several human carcinomas, including cervical, oral, and breast. More significantly, it has been recently pointed out that viral onco-proteins of EBV and high-risk HPVs can be co-present and consequently cooperate to initiate and/or amplify epithelial-mesenchymal transition (EMT), which is the hallmark of cancer progression and metastasis. This could occur by β-catenin, JAK/STAT/SRC, PI3k/Akt/mTOR, and/or RAS/MEK/ERK signaling pathways, which onco-proteins of EBV and HPVs share.     Comparison of hepatitis C virus risk factors in genotypes 1a and 3a

    Background: One of the most important causes of chronic liver disease is hepatitis C virus (HCV), which causes liver cirrhosis and hepatocellular carcinoma. To control the prevalence of the disease, knowledge and information in risk factor of HCV are required. The aim of this study was to compare the risk factors of infection between HCV patients with genotypes 1a and 3a. Methods: This is an observational analytical study. HCV patients who referred to the clinic of hepatology, Rasoul-e-Akram University Hospital from July 2015 to July 2017, were assigned to the genotype 1a and 3a. Demographic (age, sex, family history), clinical (cirrhosis, hepatocellular carcinoma) and laboratory data, history of intravenous drug and alcohol usage, and history of imprisonment were gathered and compared between two groups. All the patients completed the informed consent form. Data analysis was performed by SPSS software, version 22 (IBM SPSS, Armonk, NY, USA). P value less than 0.05 was considered statistically significant. Results: Overall, 97 HCV patients were included in this study. Mean age was 45±12 years and 78 (80) of patients were male. Among them, 58 (60) and 39 (40) had genotype 1a and 3a. respectively. History of injection drug usage was recorded in 34/39 (87) of patients with genotype 3a, and significantly higher in genotype 3a as compared to genotype 1a OR adj: 3.1, CI (1.3-6.2). Also, in this study, genotype 3a was significantly recorded in younger patients OR adj: 1.7, CI (1.2-4.1). However, cirrhosis and hepatocellular carcinoma was more common in patients with genotype 1a as compared to genotype 3a OR adj: 2.05, CI (1.6-5.4) and OR adj: 2.8, CI (1.3-5.7) respectively. Conclusion: According to the results of this study, hepatitis C virus transmission risk factors differed in genotypes 3a and 1a. Genotype 3a is found among young patients with a history of intravenous drug usage and genotype 1a in patients with cirrhosis and hepatocellular carcinoma. © 2018, Tehran University of Medical Sciences.
